Forms & Send to
Collect responses with a form, and send a copy of each one to Slack, Jira, Notion or a webhook.
Forms
A form is built in Forms, shared by link, embedded on your site, or offered by the AI agent in a conversation. Every response is kept in Komplian: it appears under Responses, and a response with an email or a phone becomes a contact in the CRM.
Send to
Under Settings → Send to a form can also deliver a copy of each response somewhere else. The response always stays in Komplian first; a destination is a copy that leaves afterwards. If the other system is down, the response is not lost: the delivery is retried, and you can see what happened next to the response.
- How many destinations one form may have depends on your plan (Growth 3, Pro 10, Enterprise 25).
- A credential answered in a form is never sent to a destination. A document is attached only where you say so.
- Each destination can be switched off without deleting it.
Destinations
Webhook
One JSON POST per response to an address you own, optionally signed so the receiver can verify it came from Komplian.
Slack message
A message in one of the channels you selected when you connected Slack, written with variables.
Jira Service Management request
A request on a service desk, raised by the service account you connected, with the fields you map and the documents you choose attached.
Jira issue
An issue in a Jira project on the same connection: project, issue type and fields.
Notion page
A row in a Notion database, with each property filled from the response.
Jira is connected once from Integrations → Jira Service Management: one click on Atlassian’s own screen with the account that should raise the requests, or a service account’s API token (site, account email and token) if you prefer one that does not belong to a person. Notion is connected from the destination itself with an integration token; share the database with that integration in Notion first.

What goes into each field is written with the same variables flows use:

Delivery status
In Responses each response carries a dot: green when every destination received it, amber while one is still on its way, red when one failed. Open the response to see each destination, the link to what was created (the Jira key, the Notion page) and, when something went wrong, Jira’s or Notion’s own words. Retry queues it again.
From a flow
The same destinations are available in a flow as the Integration action step, so a request can be raised when a lead changes status, not only when a form is answered. See Flows → Actions.
